As our Hospital Diversion Peer Support Specialist, you will be responsible for responding to individuals in hospital emergency departments and crisis stabilization units who are experiencing behavioral health crises. You will work to build trust and rapport with individuals in crisis, reduce unnecessary emergency department utilization, and promote engagement with long-term community-based care and recovery supports. This is a community-based role that covers Wake County . If you love being out and about and seeing clients in the community, you may be a great fit! We provide mileage reimbursement! SouthLight is also an approved employer for the Public Service Loan Forgiveness Program (PSLF). Responsibilities of a Hospital Diversion Peer Support Specialist Respond in-person to referrals from emergency departments (EDs), psychiatric crisis units, or behavioral health triage teams. Share personal lived experience to inspire hope and model recovery. Engage individuals in safety planning, coping strategies, and the identification of recovery goals. Assist individuals in navigating healthcare, housing, transportation, and social service systems. Collaborate with hospital staff, mobile crisis teams, and outpatient service providers to ensure smooth care transitions. Provide follow-up contacts (via phone, text, or in-person) to support continued engagement in care. Maintain accurate, timely documentation in accordance with agency and HIPAA standards. Participate in team meetings, supervision, and ongoing training. Participate in regularly scheduled complex case rounds, sharing experience and perspective from a Peer Specialist’s scope Qualifications of a Hospital Diversion Peer Support Specialist Education and Experience High school diploma or GED equivalent and be at least 18 years old. Minimum of 1 year working with the adult MH/SA population in a community setting Licensure/Certifications Active Certification in NC as Peer Support Specialist Knowledge, Skills and Abilities Required Strong written and verbal communication skills People skills including influencing, delegation, teamwork Ability to work effectively with a diverse staff, clients, and community members in a culturally competent manner Ability to multi-task and remain focused on key goals and objectives Effective representative to external partners, ability to prioritize organizational goals while initiating and influencing new relationships Experience with community-based organizations; knowledge of resources available in the community related to housing, transportation, food insecurity, inter-personal violence and more. Familiarity with needs of people in recovery and receiving substance use and mental health services Ability to learn other technological recording methods i.e. EMRs/EHRs Makes and communicates well-informed and timely decisions.
