No on-call, no weekends and noholidays required. This role requires travel across the entire state of California,approximately 85% of the time. All travel expenses are covered, andthere is likewise a base salary plus multiple incentives.

Primary Responsibilities:

Conduct in-home assessments including an evaluation of pastmedical history, review of systems, medication reconciliation,vital signs, and comprehensive physical exam Perform evidence-based practice screenings including point ofcare testing (as appropriate) Identify diagnoses to be used in care management and activemedical management in the furtherance of treatment Formulate a list of current and past medical conditions usingclinical knowledge and judgment and the findings of yourassessment Communicate findings in your assessment that will be used toinform member’s PCP of potential gaps in care Identify urgent and emergent situations and interveneappropriately Educate members on topics such as disease process, medication,and compliance You’ll be rewarded and recognized for yourperformance in an environment that will challenge you and give youclear direction on what it takes to succeed in your role as well asprovide development for other roles you may be interested in.

Required Qualifications:

Active NP license or ability to obtain by start date. Licensuremust be unencumbered, free of any open/unresolved disciplinaryactions including probation or restrictions against privilege topractice Active national NP certification or the ability to obtainnational certification in state of assignment by start date (ActiveANCC or AANP national certification in Family, Geriatrics, Adult,Adult-Gerontology Primary Care, or Adult-Gerontology Acute Carecertification) Ability to complete physical requirements of the job includinglifting a 30-pound bag in and out of the car, navigating stairs anda variety of dwelling conditions, and sit, stand and kneel toperform physical assessment Reliable transportation to complete home visit assessments

Preferred Qualifications:

1+ years of clinical experience in their highest level ofeducation, clinical setting May be requested to obtain additional licensure in othergeographic areas Experience in gerontology, cardiology, internal medicine, orendocrinology Home Health care or home visit experience Ability to travel up to 5% within licensed states if requestedfor additional incentives (with notice, based on business needs)Pursuant to the San Francisco Fair Chance Ordinance, we willconsider for employment qualified applicants with arrest andconviction records

PLEASE NOTE

Employees must be in an active regular status.

Employees must remain in role for a minimum of 12 months from thedate of hire /rehire/transfer. If an employee leaves Home andCommunity, the student loan repayments will cease. The employeemust remain in an Advanced Practice Clinician or Physician rolewithin Home and Community for 36 months to receive the full benefitof the student loan repayments.

The salary range for this role is$88,000 to $173,200 annually. Pay is based on several factorsincluding but not limited to local labor markets, education, workexperience, certifications, etc. UnitedHealth Group complies withall minimum wage laws as applicable.
