We’re looking for a dedicated Nurse Practitioner with experience in Addiction Medicine OR Psychiatric Mental Health Nurse Practitioner (PMHNP) who is passionate about mental health, addiction recovery, and making a difference in patients’ lives. Position Role As a Nurse Practitioner at Pathway Healthcare, you will play a crucial role in evaluating and treating patients with mental health and addiction issues. You’ll work independently and collaboratively with an interdisciplinary team, using the latest evidence-based practices to counsel, treat, and support individuals on their path to recovery. Key Responsibilities
- Conduct thorough assessments, identify diagnoses, and develop treatment plans for individuals with addiction and psychiatric conditions.
- Order, interpret, and evaluate diagnostic tests based on patient needs and provide informed diagnoses.
- Consult with physicians regarding abnormal test results, offering patient feedback and education as needed.
- Educate patients on their conditions and assist them in setting realistic care goals.
- Accurately document patient interactions in the electronic medical record (EMR).
- Prescribe appropriate pharmacologic, non-pharmacologic, and psychotropic medications or recommend alternative therapies as necessary.
- Educate and counsel patients about medication adherence and long-term health.
- Work within an interdisciplinary team to provide holistic care addressing both medical and behavioral health needs.
- Conduct follow-up appointments to monitor treatment progress and make necessary adjustments.
- Provide consultations on lab results and make informed decisions regarding care plans. Qualifications
- Active Nurse Practitioner (NP) licensure in good standing.
- Current DEA license.
PREFERRED PMHNP OR
experience in OBOT (Office-Based Opioid Treatment), behavioral health, primary care, or community health.
- Strong written and verbal communication skills.
- Commitment to high standards of patient care and team collaboration.
