Nurse Practitioner Confidential Montgomery, AL 36106 Nurse Practitioner Job Description Job Summary We are seeking a dedicated and compassionate Nurse Practitioner/ PA to join our healthcare team. We are expanding our team to include wound care and IV services in Montgomery. This position will require visits to LTC facilities and homes for patients with Home Health. Practitioner must collaborate with care teams and other healthcare professionals. Position requires an emphasis on documentation and ability to work independently. Duties Conduct comprehensive patient assessments, including medical histories and physical examinations. Manage patients in Nursing Homes , Home Health setting and clinic setting. Develop and implement individualized care plans based on evidence-based practices and payor requirements Perform procedures such as sterile processing, infection control measures, and critical care interventions. Perform IV’s when appropriate Monitor vital signs and interpret diagnostic tests to inform treatment decisions. Provide telehealth services to enhance patient access to care. Educate patients and their families about wound management, health management, disease prevention, and wellness strategies. Collaborate with interdisciplinary teams to ensure coordinated patient care. Maintain accurate documentation of patient encounters in compliance with healthcare regulations. Desire to work with Wound Care patients. Requirements Master’s degree in Nursing or a related field from an accredited program. Current Nurse Practitioner certification and state licensure. Proficiency in medical-surgical nursing practices. Experience or knowledge in geriatrics, wound care, SNF, home health setting Familiarity with ICD-10 coding for accurate documentation of diagnoses. Strong skills in infection control protocols and critical care management. Excellent communication skills, both verbal and written, with the ability to engage effectively with patients and families.
