PatientConnect is one-time comprehensive visit designed for patients whohave recently been hospitalized and are in a skilled rehabfacility. The purpose of this visit is to ensure a smoothtransition from rehab to home and to prevent readmission. Duringthis visit you will capture STAR measures, educate the patient, anddocument all their acute and chronic diagnoses. This positionprovides the NP multiple opportunities each day to use theirexperience and education in varying settings to provide guidanceand compassion to a vulnerable population during a difficulttransition in their healthcare. The NP will not be managing carebut will be communicating pertinent findings to the attending andfacility staff. You will not bear the heavy responsibility fororders, treatments, and follow up. You will have the autonomy youdeserve, which shows your value and ability to work independently.

Primary Responsibilities:
- Performcomprehensive assessments and document findings in aconcise/comprehensive manner that is compliant with documentationrequirements and Center for Medicare and Medicaid Services (CMS)regulations
- Responsible for ensuring that all diagnoses areICD10, coded accurately, and documented appropriately to supportthe diagnosis at that visit
- The APC is responsible for ensuringthat all quality elements are addressed and documented
- The APCwill do an initial medication review
- Must attend and complete allmandatory educational and computer/virtual training requirements
- Travel between care sites mandatory
- Care Coordination
- Communicate with the patient and necessary team members regardingany medical concerns at the time of your visit
- ProgramEnhancement Expected Behaviors
- Exhibit original thinking andcreativity in the development of new and improved methods andapproaches to concerns/issues
- Function independently andresponsibly with minimal need for supervision
- Ability to enteravailable hours into web-based application, at least one monthprior to available work time
- Demonstrate initiative in achievingindividual, team, and organizational goals and objectives
- Participate in SCC quality initiatives

Required Qualifications:
- Certified Nurse Practitioner through anational board

Graduate of an accredited master’s degreein Nursing (MSN) program and board certified through the AmericanAcademy of Nurse Practitioners (AANP) or the American NursesCredentialing Center (ANCC), Adult-Gerontology Acute Care NursePractitioners (AG AC NP), Adult/Family or Gerontology NursePractitioners (ACNP), with preferred certification as ANP, FNP, orGNP

Graduate of an accredited Physician Assistant degreeprogram and currently board certified by the National Commission onCertification of Physician Assistants (NCCPA)
- Active andunrestricted APC license in the State of Connecticut
- Currentactive DEA licensure/prescriptive authority or ability to obtainpost-hire, per state regulations (unless prohibited in state ofpractice)
- Driver’s license and access to reliable transportationthat will enable you to travel to client and/or patient siteswithin a designated area
- Availability to work 10 hours per week
- Ability to gain a collaborative practice agreement, if applicablein your state
Preferred Qualifications:
- 1 years of hands-on postgrad experience within Long Term Care
- Ability to move a 30-poundbag in and out of car and to navigate stairs and a variety ofdwelling conditions and configurations
- Understanding ofGeriatrics and Chronic Illness
- Understanding of Advanced Illnessand end of life discussions
- Proficient computer skills includingthe ability to document medical information with written andelectronic medical records
- Ability to develop and maintainpositive customer relationships
- Adaptability to change Thisrole’s wage is based on a per visit amount that falls in the rangeof $90 to $155, and actual earnings will vary based on number ofvisits performed.
