Open to Nurse Practitioners (FNP,ANP, AGNP) and Physician Assistants (PA)
Location :
Homecare basedin Eastern Connecticut. Locations can be flexible based on thecandidate’s home.
Setting :
Geriatric Medicine in the home setting,working within a multi-disciplinary care team

Nurse Practitioners and PhysicianAssistants Role Scope:
Under established guidelines and incollaboration and/or supervision with the practice physician(s),Nurse Practitioners and Physician Assistants perform routinephysical examinations, and formulate medical, educational, andpsychosocial plans to treat patients. In consultation with thephysician(s) and care team members, Nurse Practitioners andPhysician Assistants develop plans for further evaluation of healthneeds, e.g., referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriatetreatment for problems within the scope of the provider’s practiceas designated by protocols and identifies problems outside of theirscope of practice and refers appropriately to physicians.
Qualifications Requirements :
Licensure:
Unrestricted State ofConnecticut Medical License (or eligible)
Training:
APRN (Family,Adult/Geri Nurse Practitioner) or
PA Experience:
At least two yearsof post-graduate experience as a Nurse Practitioners (APRN) orPhysician Assistants (PA) New grads with relevant nursingexperience are welcome to apply!
