Field Nurse Practitioner, Care Connections Locations:
Middletown, New Haven, Litchfield, Willimantic, Rockville, New London, CT Compensation:
$100,000 – $160,000 annually Are you a compassionate and adaptable Nurse Practitioner eager to make a significant impact on underserved populations? We are seeking dedicated Field Nurse Practitioners to join our Care Connections team. This role focuses on providing essential screening and preventive primary care services directly to patients in their homes, community settings, and nursing facilities. Position Overview As a Field Nurse Practitioner, you will primarily work in non-clinical settings, delivering comprehensive medical care to a diverse patient population, including adults, geriatrics, pediatrics, and women’s health. This role emphasizes initial assessments, focusing on gaps in care and preventative services, with no ongoing follow-up care required. Each visit is approximately 45 minutes, and you will typically complete 6-9 visits per day, depending on your chosen shift. You’ll be charting in the field using Epic EMR , representing the organization during every interaction. Key Responsibilities Provide general medical care and care coordination to various patient populations (adults, women’s health, pediatric, and geriatric). Perform comprehensive evaluations, including history and physical exams, to identify gaps in care and conduct preventative assessments. Address chronic and acute primary care complaints, accurately assessing medical urgency. Establish and document reasonable medical diagnoses. Seek specialty consultations as appropriate. Order and perform pertinent diagnostic laboratory and radiology testing judiciously, working effectively within an environment of limited resources. Recognize when patient needs exceed your scope of knowledge and when physician oversight is necessary. Create and implement medical plans of care. Schedule patient appointments for subsequent visits when appropriate. Provide post-discharge coordination to help reduce hospital readmission rates and emergency room utilization. Conduct face-to-face, in-person visits in a variety of settings, including homes, skilled nursing facilities, and public locations. Potentially perform face-to-face visits via alternate modalities based on business needs, leadership direction, and state regulations. Order bulk laboratory orders to target specific member populations. Participate in alternating on-call coverage to triage urgent lab results and pharmacy inquiries, developing appropriate plans of care. Engage in community-based “Pop Up Clinics” to build relationships and address healthcare gaps. Drive regularly (up to 120 miles daily) within your assigned region. Extended Mileage Special Project days may involve drives beyond 120 miles and potentially overnight hotel stays. Obtain and maintain cross-state licensure as required by business needs. Collaborate with fellow Nurse Practitioners to develop efficient and effective best practices. Actively participate in regional meetings. Prescribe medications and perform procedures as appropriate. Perform timely documentation in medical records using an electronic medical record system. Occasionally, walk flights of stairs while carrying up to 50 lbs. of equipment. Schedule/Shift Options Our organization offers flexible scheduling to support work-life balance: Option 1 (4×10): Work four 10-hour days (Monday-Friday) from 8:00 AM-7:00 PM, including one Saturday per month. This option involves 8-9 visits per day. Option 2 (5×8): Work five 8-hour days (Monday-Saturday). This schedule includes three days from 8:00 AM-5:00 PM and two days from 10:00 AM-7:00 PM. You can choose which five days you work and which two days you work late, maintaining a consistent schedule week-to-week. This option involves 6-7 visits per day. Qualifications Must-Have Skills/Qualifications Active and unrestricted Adult or Family Nurse Practitioner license . Comfortable with 100% home visits . Must genuinely enjoy fieldwork and driving . Must reside in or near (within 20 minutes of) the assigned territory. Ability to pass a clear background check and be available for mandatory 5-week onboarding and training . Nice-to-Have Skills Previous home health or hospice experience. Experience working with vulnerable or underserved populations. Soft Skills/Attributes Compassionate and adaptable .
