Location:

Danbury or Greenwich, CT Position type: Full-Time Position style: Hybrid (In-Person and Telehealth)

Work Schedule:

Clinic hours : Monday to Friday 8:30 AM to 5:30 PM Role We are looking for compassionate Psychiatric Providers•Psychiatrist (MD / DO) and Mid-Level Providers (PMHNP or others with Mental Health / Psychiatric Experience) to join our team of highly skilled, outpatient Mental Health care providers and clinicians. The ideal candidate has experience providing a range of psychiatric assessment, diagnostic, therapeutic, and case management services for individuals, couples, groups, and/or families. We are looking for culturally competent candidates with training and experience for a wide range of patient populations with varied mental health illnesses. Our providers are responsible for intaking and assessing patients, evaluating patient’s medical and clinical history, and establishing an individualized treatment plan with pharmaceutical care if clinically appropriate. Hybrid Role•

Telehealth and In-Person Responsibilities:

Build a positive, therapeutic, and trusting clinical relationship with patients. Evaluate the patient’s medical and pharmaceutical history, assess the presenting symptoms, and treat mental health illnesses as clinically appropriate. Establish an individualized treatment plan, catered to the specific patient. Provide ongoing engagement and treatment as outlined by the care plan and necessary to the patient for successful clinical outcomes. Prescribe clinically appropriate medicines as necessary to facilitate improvement in mental health diagnoses. Provide case management and coordination of care services as necessary. Continue to evaluate patients’ treatment progress and care plan. Consistent, thorough, and timely documentation for treatment plans and ongoing care. Build an outcomes-focused, collaborative care environment for colleagues and patients. Maintain patient confidentiality and an appropriate clinical environment for the patient. Attend and engage in supervision with clinical provider team.

Requirements:

Psychiatric / Mental Health experience. Actively licensed to practice and prescribe in Connecticut.
