Jacksonville, Florida Advanced Practice Provider 46187 Overview Job Duties An Advanced Practice Provider I (APP I) is a direct patient care provider licensed in the State of Florida and/or Georgia and provides medical services to patients as defined by their scope of practice and under the supervision of a physician licensed to practice medicine in the State of Florida/Georgia.

Clinical Area:

Primary Outpatient Care in Community Health & Family Medicine and General Pediatrics. May perform minor Primary care procedures. Essential Functions

  • Obtains patient histories, performs physical examinations and/or diagnostic and therapeutic procedures, as well as orders, interprets, and evaluates diagnostic tests.
  • Formulates comprehensive health management plans for patients in collaboration with physicians, pharmacists, and other medical professionals to include progress notes, narrative discharge summaries, consults, etc. Facilitates patient participation in making health care decisions.
  • Reviews, prescribes and/or recommends medications and treatments to patients
  • Provides timely and appropriate referrals to patients in need of further medical care and services
  • Educates and counsels patients/families regarding their health care, including routine health maintenance
  • Enters pertinent clinical information in the patient’s medical record in a timely and accurate manner and in accordance with established compliance billing guidelines. Documents and closes open encounters, MyChart messages, and other EMR communications in accordance with institutional requirements.
  • May provide education and instruction.
  • Call coverage may be required specific to departmental needs.

Temperament Must be able to perform under stress when confronted with emergency, critical, or unusual situations. Must be capable of dealing with periodic cyclical workload pressures and levels of responsibility. Required to make independent judgments without supervision. Must be able to make generalizations, evaluations, or decisions based on sensory or judgmental criteria. Must have the adaptability to perform a variety of duties, often changing from one task to another of a different nature without loss of efficiency or composure. Requires the ability to work with people beyond giving and receiving instructions. Qualifications Skills, Knowledge, Abilities

  • Knowledge of regulatory agency standards (e.g. Joint Commission, HRS), Nursing Standards and Florida Nurse Practice Act.
  • Capacity to assess patient health and measure/record physiologic and growth indices.
  • Knowledge of acute and chronic care protocols, administration of medications, delivering physical care following treatment plan.
  • Knowledge of health promotion principles, techniques and patient learning measurements.
  • Strong interpersonal and communication skills with the ability to work efficiently with a wide range of constituencies in a diverse community.
  • Knowledge of regulatory requirement (billing compliance) and applicable laws.
  • Ability to develop health education programs and materials.
  • Ability to make responsible decisions within the scope of a dependent medical practitioner.
