JOB PURPOSE

We are seeking a Nurse Practitioner/PhysicianAssistant to work in collaboration and consultation withphysicians, staff RN’s and other health care professionals, toprovide high touch, compassionate care to patients and families andprovides care coordination and care management activities.

Practices within the context of collaborative management with thephysician(s) in diagnosing, managing, and preventing acute andchronic illness and disease, and promoting wellness. NursePractitioners have an advanced nursing practice that includesindependent nursing functions based on nursing standards of careand a role in medical management working within a collaborativeagreement with a physician(s).

KEY RESPONSIBILITIES

1. Provenability to perform comprehensive, adult/geriatric admission historyand physical exam 2. Demonstrates ability to identify, diagnose anddocument episodic, acute and chronic disease in adults 3.Demonstrates ability to recognize conditions, characteristics, andbehaviors that place seniors at risk 4. Proven ability to pursuefeedback, continuing education and professional development 5.Ability to effectively reconcile medications 6. Demonstratesability to identify and document the needs of clinically complexpatients 7. Work collaboratively with physicians, case managers,social workers, family members, key care givers and any appropriateancillary medical personnel as appropriate 8. Participates inQuality Assurance Performance Improvement (QAPI) activities 9.Assesses the medical necessity/effectiveness of ancillary servicesto determine the appropriate initiation of benefit events andcommunicate the process to providers and appropriate team members10. Coordinates care as member’s transition through differentlevels of care and care settings, ensures that the ICP iscommunicated immediately and/or no later than 24 hours to thereceiving care setting

MINIMUM EDUCATION REQUIRED

Graduate of an accredited Adult orFamily Nurse Practitioner Master’s Degree program or PhysicianAssistant Studies

MINIMUM EXPERIENCE REQUIRED

Minimum 2 yearsclinical experience as a Nurse Practitioner or Physician Assistantin geriatric, adult or family practice setting. Proven ability toperform comprehensive adult/geriatric physical exams.

MINIMUMLICENSURE/CERTIFICATION REQUIRED BY LAW

Current and unrestricted, RN and NP licensure, or PA licensurein state of practice. Board certification by AANP or ANCC, inAdult, Family Practice and Geriatrics or Adult-Gerontology PrimaryCare. Current CPR certification — Active DEA license Must have an active national provider number (NPI) and anactive Medicare provider number Must be eligible to participate in the Medicare program as aprovider and registered

ADDITIONAL QUALIFICATIONS

(Preferredqualifications) Graduate of an accredited Geriatric, Adult orFamily Nurse Practitioner Master’s Degree program. Previous longterm care experience strongly preferred.

KNOWLEDGE, SKILLS,ABILITIES 1.

Pertinent Clinical Work Experience which is relevantto population being served. 2. Reviews and interprets the medicaland social history of members 3. Interprets laboratory data,recommend appropriate follow-up, reconcile medications 4. Managesboth medical and behavioral chronic and acute conditionseffectively in collaboration with a physician or specialty provider5. Synthesizes information and easily diagnoses and creates acomprehensive plan of care. 6. Complete all regulatory requiredmonthly, quarterly, and/or annual visits and acute visits asneeded. 7. Maintain appropriate and compliant documentation withinour EMR (electronic medical record) software 8. Ensures alldiagnoses are ICD-10 coded accurately and documented appropriatelyto support the diagnosis at that visit 9. Reviews orders andinterventions for appropriateness and response to treatment toidentify the most effective plan of care that aligns with themember’s needs and wishes 10. Participate in family conferences andprovide advanced care planning and education when needed 11.Address any pharmacy recommendations or pharmacist communicationsregarding residents 12. Providers are expected to be present inassigned facilities during work hours to ensure that facilitieshave adequate face-to-face coverage to meet resident care andfacility needs 13. Practice ethically and in accordance with theScope and Standards of Practice of their profession and BoardCertification 14. Rotating on-call responsibilities and after-hourcoverage is required. 10.
