Responsibilities Conduct comprehensive assessments of patients with acute and chronic wounds. Develop individualized care plans based on assessment findings and patient needs. Perform wound debridement, dressing changes, and other necessary procedures. Provide education to patients and families regarding wound care management and prevention strategies. Collaborate with other healthcare professionals to ensure coordinated care for patients. Monitor patient progress and adjust treatment plans as necessary. Participate in quality improvement initiatives related to wound care practices. Maintain accurate documentation of patient assessments, interventions, and outcomes. Qualifications Master’s degree in Nursing with specialization as a Nurse Practitioner. Current Nurse Practitioner license in the state of practice. Strong knowledge of physiology related to wound healing and management. Experience in acute care settings preferred; outpatient experience is a plus. Ability to provide home care services as needed for patients requiring ongoing support. Excellent communication skills with the ability to educate patients effectively.
