Exempt About the Job The IHA Per Diem Nurse Practitioner/Physician Assistant performs in-person, no-cost annual preventative health risk assessments for Medicare, Medicaid, and Affordable Care Act (ACA) members. These assessments are conducted within the member’s living space to meet health plan expectations. Responsibilities Conduct 25-30 visits per month in Indian River County. Perform high-quality assessments to ensure member satisfaction and meet health plan standards. Complete chart documentation within 24 hours of each visit and respond to queries within 48 hours . Maintain excellent communication with clinical leadership staff. Participate in periodic training sessions and meetings as required. Comfortably enter personal living spaces of members scheduled for assessment. Lift up to 20 lbs. and bend or kneel for extended periods when necessary. Schedule & Compensation Availability provided once a month for 25-30 hours/slots. Appointments available Sunday-Saturday, 7 AM-7 PM with 60-75-minute slots per assessment. Compensation at market rate per completed assessment , with mileage reimbursement beyond 30 miles per day at the current IRS rate . Required Skills & Qualifications Education & Experience Master’s or Doctorate degree from an accredited NP program (FNP, AGNP, ANP) or Master’s degree from an accredited PA program. Home health experience required within the past 5 years . 1 year of in-home assessment experience highly preferred . Competencies Strong computer skills and proficiency with electronic medical records (EMR) . Knowledge of risk adjustment models for CMS and ACA programs. Ability to document findings accurately and thoroughly . Effective verbal and written communication with members and clinical teams. Maintain patient confidentiality in accordance with HIPAA regulations . Ability to adapt to changing conditions and work responsibilities . Certifications & Licensure Current RN and NP or PA licensure in the state of practice. Board-certified by ANCC, AANP, or NCCPA. Current Medicaid ID and Medicare PTAN number . BLS certification required. Valid driver’s license and proof of car insurance for state appointments.
