The ideal candidate will be responsible for assessing, diagnosing, treating, and managing a variety of medical conditions for patients who are homebound, elderly, or have mobility challenges. Key responsibilities include performing physical exams, ordering and interpreting diagnostic tests, prescribing medications, developing personalized care plans, and offering health promotion and disease prevention advice. The NP will work independently and in collaboration with a healthcare team to ensure optimal patient outcomes in a patient-centered environment. Very positive, friendly, supportive work environment.
