Geriatrics Full Job Description Job Overview:

We are seeking a dedicated Family, Adult or Acute Care Nurse Practitioner who shares our passion for excellence and commitment in providing high quality patient care. We provide primary care in independent living, assisted living and memory care facilities in southwest Florida.

Duties:

• Evaluates new and existing patients by performing a history and physical and interpreting laboratory and diagnostic studies for acute and chronically ill patients.
• Orders and interprets diagnostic and laboratory tests related to the patients needs.
• Prescribes appropriate pharmacologic medications.
• Communicates treatment plans between patient, family and facility team.
• Educates patients on health maintenance and disease prevention
• Collaborates with multidisciplinary team members.

Requirements:

• Graduate of an accredited nurse practitioner program
