Beatriz Coccaro-Word On Call Wound Care LLC provides the services of our nurse practitioners who will evaluate and treat patients in the comfort of their homes or if they are residing at a skilled or long-term care nursing facility. Our mission is to achieve faster healing of chronic wounds and to prevent the development of facility-acquired pressure injuries by providing evidence-based treatments with an emphasis on value delivery and personalized compassionate care.
GENERAL JOB DESCRIPTION
On Call Wound Care, LLC desires to secure the provision of an Advanced Practice Registered Nurse (APRN) who works autonomously within their scope of practice but in collaboration with physicians and other clinical staff to provide and manage advanced wound care in a residential or skilled nursing facility setting.
MAJOR DUTIES AND RESPONSIBILITES 1.
Provision of hands-on care to patients, as directed by On Call Wound Care nurse practitioner evaluation and treatment protocols. 2. Debridement of wounds by sharp, enzymatic, or autolytic methods completed safely during the patient visit. 3. Application of topical wound care and associated wound management products. 4. Recommendation of associated services to improve wound healing outcomes. This includes but is not limited to, labs, radiologic tests, vascular tests, specialist referrals, home health services referrals for skilled nursing and physical therapy, negative pressure wound therapy devices, and orthotics. 5. Interpretation of diagnostic tests and recommendation of appropriate follow-up to same. 6. Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license, and as per the collaborative agreement with the supervising physician. 7. Risk assessment for Members with Diabetes, as per the American Diabetes Association (ADA) guidelines. 8. Patient education and management not only to patients but also to family members, caregivers, and other providers involved in the care. 9. Education of the Primary Care Physician (PCP), Case Manager (CM), Home Health nurse (HHN), physical therapist (PT), or referring Physician on current wound management practices. 10. Provide patient updates to PCP, CM, HHM, PT, and Physicians via electronic faxing and verbal or written communications. 11. Communicates in a clear and understandable manner using appropriate verbal and non-verbal styles. 12. Actively participates in departmental activities to improve training methods and achieve company goals. 13. Assists with one-on-one training and coaching sessions as assigned. 14. Attends required meetings and participates as a committee member as requested. 15. Assists with special projects as needed. 16. Maintains strict confidentiality. 17. Manages patient’s schedules and is responsible for contacting patients to inform them of scheduled initial consultations and follow-up appointments 18. Accurately and thoroughly document patient encounters on On Call Wound Care electronic medical record for each patient visit and electronically fax notes to referring providers and other clinical staff involved in the patient’s care 19. Communicate effectively with the patient’s family, the medical power of attorney, the primary care provider, and all individuals involved in the patient’s care. 20. Other duties as assigned.
MINOR DUTIES AND RESPONSIBILITIES QUALIFICATIONS FOR THE JOB
Education:
Graduate of Master’s of Nursing degree from an accredited school of Nursing Certification from the American Nurses Credentialing Center – preferred but not required Wound care certification from the American Board of Wound Management or from The Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB)- preferred but not required BLS certification Current, active, unrestricted license as an APRN in the state of Florida.
Experience:
Two years minimum of wound care experience
Other:
Ability to practice autonomously. Ability to lift 50 lbs. or more. Must have a full range of mobility. Must be able to reposition patients. Valid and unrestricted driver ‘ s license with a clean driving record Excellent verbal and written communication skills in a professional environment. Ability to set priorities while managing multiple tasks. Detail oriented with excellent organization skills. Uploading data to One Drive- Preferred Strong Writing Skills- need to be proficient in spelling, grammar, and syntax. Strong Typing Skills-Accuracy and proficiency are required Strong Computer Skills-must be able to operate and use certain computer software programs to complete their assignments. This includes word processors, like Microsoft Word, and audio file editing equipment. Must be focused and have strong attention to detail.
