Job Summary The part-time Nurse Practitioner plays a vital role in delivering high-quality, patient-centered care within our medical center. Under the collaborative supervision of physicians, the NP performs comprehensive physical examinations, diagnoses and treats acute and chronic conditions, orders and interprets diagnostic tests, prescribes medications, and educates patients on preventive health strategies. The NP ensures continuity of care through effective communication with multidisciplinary teams and maintains a strong focus on improving health outcomes and patient satisfaction. This position requires a commitment to excellence, adherence to clinical protocols, and alignment with our organization’s mission to provide compassionate and accessible healthcare. Job Responsibilities Deliver high-quality, patient-centered care to elderly patients, including routine check-ups, preventive care, and management of chronic diseases. Conduct thorough evaluations, including physical examinations, medical histories, and diagnostic tests to assess patients’ health status and develop individualized treatment plans. Develop and implement evidence-based treatment plans for a variety of conditions, including age-related illnesses and comorbidities. Collaborate with other healthcare professionals, including APRN’s, specialists, and social workers, to ensure comprehensive and coordinated care. Educate patients and their families about health conditions, treatment options, and lifestyle changes to promote better health outcomes. Oversee the management of chronic conditions such as diabetes, hypertension, and heart disease, including medication management and lifestyle recommendations. Promote preventive health measures, including vaccinations, screenings, and health maintenance activities. Maintain accurate and up-to-date medical records in accordance with organizational policies and regulatory requirements. Participate in quality improvement initiatives and contribute to the development of best practices in geriatric care. Education and Experience Highest Level of Accredited Education. Current and unrestricted APRN license in the state of Florida. National board certification (if applicable). Current DEA registration (If applicable) Minimum of 2 years of experience working with Medicare or geriatric patients in a clinical setting. Knowledge, Skills & Proficiencies Strong diagnostic and clinical skills, with the ability to manage complex medical conditions in an aging population. Excellent interpersonal and communication skills, with the ability to build rapport with patients and their families. A compassionate approach to patient care, with a commitment to improving the quality of life for elderly individuals. Ability to work effectively within a multidisciplinary team and contribute to a collaborative care environment. In-depth knowledge of adult and geriatric care, especially within the Medicare population. Proficient in electronic medical records (EMR/EHR) systems. Strong diagnostic and clinical decision-making skills. Excellent interpersonal, communication, and counseling skills. Ability to maintain patient confidentiality and comply with HIPAA standards. Effective time management and ability to handle multiple tasks. Bilingual (English/Spanish) preferred. Physical Requirement/Working Environment Work in a clinical or healthcare setting, such as a medical office or clinic Interact directly with patients and team members. Fast paced and requires multitasking, including both clinical and administrative duties Adhere to health and safety protocols, including the use of personal protective equipment (PPE) and infection control procedures. Handling medical equipment.
