As a valued member of our team, you’ll provide preventive care assessments to patients in the comfort of their homes. These comprehensive evaluations help identify and reduce health risks, while connecting patients to additional support services and follow-up care as needed. Our holistic approach ensures we address the physical, mental, and social health needs of our patients, creating a seamless and impactful healthcare experience.
Key Responsibilities In-Home Assessments :
Conduct thorough health evaluations, including reviewing past medical history, vital signs, evidence-based screenings, and comprehensive physical exams.
Care Coordination:
Proactively identify health risks and gaps in care, facilitating clear and effective communication between patients and their primary care providers.
Note:
This role does not involve treatment or prescribing.
Education & Engagement :
Council patients on topics such as chronic disease management, medication adherence, and preventive care strategies to promote individual well-being.
Travel & Flexibility:
Conduct assessments through local or regional travel, with all related expenses fully covered. Enjoy flexible scheduling that supports a healthy work-life balance, with no on-call duties or holiday work required.
Accurate Reporting :
Document assessment findings promptly and accurately, ensuring compliance with clinical standards and timely submission of reports. Qualifications Education & Certification :
Active Medical Doctor (MD/DO), Nurse Practitioner (NP), or Physician Assistant (PA) license in your state of practice. National certification from CAQH (required).
For NPs:
Master’s or Doctorate in Nursing from an accredited program.
For PAs:
Graduation from an accredited PA program.
Experience :
Minimum of 1 year of clinical experience in primary care, internal medicine, geriatrics, or a related field. Experience in preventive care, chronic disease management, or home health preferred.
Skills & Competencies :
Strong clinical assessment and diagnostic skills. Proficiency in electronic health records (EHR) documentation. Excellent communication and interpersonal skills. Ability to work independently and adapt to varying environments.
Other Requirements :
