Approximately 7 – 8 months Job Overview The Senior Community Care (SCC) Program is a longitudinal, integrated care delivery program designed to coordinate the delivery of clinical care in a facility setting. The Advanced Practice Clinician (APC) within SCC provides care to our highest-risk health plan members as part of an interdisciplinary team. Primary Responsibilities Conduct comprehensive assessments Manage acute and chronic medical and behavioral conditions effectively Ensure accurate and complete documentation, including ICD-10 conditions Communicate and collaborate with the interdisciplinary care team Facilitate advanced illness and advanced care planning discussions Provide patients and caregivers with counseling and education Required Qualifications Active and unrestricted license in the state of residence or ability to obtain by start date

Education:

NP:

Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program

PA:

Graduate of an accredited Physician Assistant (PA) degree program

Certification:

NP:

Board-certified by the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) with certification in one of the following specialties:

Family Nurse Practitioner Adult Nurse Practitioner Gerontology Nurse Practitioner Adult-Gerontology Acute Care Nurse Practitioner PA:

Board-certified by the National Commission on Certification of Physician Assistants (NCCPA) Current, active DEA licensure/prescriptive authority Ability to complete physical requirements of the role, including: Lifting a 30-pound bag Navigating stairs and various dwelling conditions Sitting, standing, and kneeling for physical assessments Reliable transportation Ability to gain a collaborative practice agreement , if applicable in the state Full

vaccination required Preferred Qualifications 1+ year of clinical experience (long-term care preferred) Experience managing complex behavioral, social, and functional needs Geriatrics & Chronic Illness expertise Knowledge of advanced illness and end-of-life care discussions Proficiency in electronic medical records (EMR) systems
