Advanced Practice Nurse Grace Healthcare Solutions, Inc. Belle Glade, FL 33430 Job Overview We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide high-quality, patient-centered care to individuals and families across their lifespan. This role requires a strong commitment to health promotion, disease prevention, and management of acute and chronic conditions. The FNP will work collaboratively with other healthcare professionals to ensure comprehensive care delivery. Responsibilities Conduct thorough assessments of patients, including medical history and physical examinations. Diagnose and manage acute and chronic illnesses, providing appropriate treatment plans. Perform procedures such as phlebotomy, coding for diagnoses, and managing Durable Medical Equipment (DME). Provide health coaching to patients to promote wellness and guide them in managing their health conditions. Collaborate with interdisciplinary teams to develop individualized care plans that meet the needs of patients. Educate patients and families on health maintenance, disease prevention, and treatment options. Maintain accurate patient records in accordance with standards and regulations, including ICD-9 coding. Stay current with best practices in acute pain management and medical-surgical care. Requirements Master’s degree in Nursing from an accredited program. Current certification as a Family Nurse Practitioner (FNP) by a recognized certifying body. Valid state nursing license with prescriptive authority. Experience in acute care settings is preferred but not mandatory. Strong clinical skills in assessment, diagnosis, and treatment planning. Ability to work effectively in a fast-paced environment while maintaining high standards of care. Excellent communication skills with the ability to educate patients effectively. Familiarity with electronic health records (EHR) systems is advantageous.
