The ideal candidate will provide comprehensive patient care across various age groups, focusing on preventive health measures, chronic disease management, and acute care. The FNP will work collaboratively with physicians and other healthcare professionals to ensure high-quality patient outcomes while maintaining a supportive and empathetic environment. Duties Conduct thorough patient assessments, including medical history and physical examinations. Develop and implement individualized care plans based on patient needs. Perform diagnostic tests and interpret results to guide treatment decisions. Manage chronic conditions and provide education on disease prevention. Administer vaccinations and perform minor procedures as needed. Document all patient interactions accurately in medical records. Utilize telehealth platforms to provide remote consultations when necessary. Collaborate with multidisciplinary teams to coordinate comprehensive care, including memory care and behavioral health services. Provide guidance on durable medical equipment (DME) needs for patients. Monitor vital signs and perform IV insertion as required. Stay current with best practices in toddler care and dermatological procedures such as dermal fillers. Qualifications Master’s degree in Nursing with a focus on Family Nurse Practitioner program. Current state licensure as a Family Nurse Practitioner. Certification by a recognized national board (e.g., AANP or ANCC). Proficiency in medical documentation practices and electronic health records (EHR). Experience in telehealth services is preferred. Knowledge of memory care strategies and behavioral health management is advantageous. Strong clinical skills in vital signs assessment, IV insertion, and familiarity with DME protocols. Excellent communication skills with the ability to build rapport with patients of all ages.
