Job Description The Nurse Practitioner (NP) will be responsible with the appropriate MD supervision for the day-to-day medical management of all patients/clients admitted to the organization program or services. Services include routine H&P’s, orders for laboratory tests and diagnostic services, ordering and managing medication, ordering therapeutic diets, and conducting general health assessments. Key Responsibilities Completed history & physical (H&P) for all patients admitted within the prescribed time required by the organization. H&P requires interviews with patients and as needed with family members to obtain historical information. Orders, monitors, and reviews laboratory tests. Acts upon the results of the tests and makes changes to the plan of care as needed. Orders, monitors, and reviews response to medication orders. Acts upon the results of response to medication regime and makes changes to the plan of care as needed. Reviews reports of adverse reactions. Educated patients on possible side effects. Orders, monitors, and reviews therapeutic diet. Acts upon reports of patient reaction and/or compliance to such diets and makes changes to the plan of care as needed. Writes orders for routine patient management including vital signs, weight, input, and output as needed and monitors data collected. Provides general health counseling to all patients under his/her care. Conducts suicide risk assessment utilizing standardized, evidence-based tools as needed. Identifies need for, and orders medical specialty referrals as needed. Acts upon the results of referrals. Provides input to the interdisciplinary treatment team on health priorities, needs, and issues. Develops medical management plan of care as needed. Enter all patient interactions, orders, and communications in the electronic medical record. Orders, monitors, and or refers patients for needed or required immunizations. Completes any required report to local health authorities regarding any reportable infectious disease. Completes discharge and transfer orders and ensures timely and effective transition to the next level of care provider. Communicates with the next provider if needed. Completes medication reconciliation process on all admissions and discharges. Write activity restriction orders as needed. Conducts utilization medical review calls as needed. Consults with medical director or designee as needed to ensure safe and effective care, treatment, or services. Practices compliance with all National Patient Safety Goals. Triages patients by severity of illness before, during, and after emergency to determine treatment priorities. Establish rapport, systematically gather data, determine the readiness of the individual for treatment and change, and apply accepted criteria for diagnosis of substance use disorders. Screen for psychoactive substance toxicity, intoxication, and withdrawal symptoms. Screen for danger to self or others Screen for co-occurring mental health issues. Analyze and interpret data to determine treatment recommendations and priorities. With the individual served, formulate mutually agreed-upon, measurable treatment goals and objective All other duties as assigned. Qualifications Active and unencumbered Advance Practice Registered Nurse (APRN) license in the State of practice. CPR certification required. Master’s degree in Nursing required. Doctorate of Nursing preferred. Nurse Practitioner and Registered Nurse licenses in the state. Board Certification as Family Nurse Practitioner required or Dual Certification Nurse Practitioner preferred 2-4 years’ experience as Nurse Practitioner in a substance abuse, mental health, or behavioral health setting. Detox experience is preferred.
