Overview:

We’re lookingfor a Hospice Nurse Practitioner to join our team. You will reportdirectly to the Executive Director or Administrator. You will beassigned to a location to perform Face-to-Face (F2F) encounters andbe responsible for gathering clinical information to support thehospice diagnosis and prognosis. These findings are thencommunicated to the hospice physician responsible for certifying orrecertifying the patient’s hospice eligibility. -The scope ofpractice for the hospice NP is limited to those tasks necessary tocomplete hospice F2F encounter visits. Performs and completes physical assessments for the F2Fencounter within the regulatory time frames established by CMS Provides clinical findings, including review of the medicaltreatment plans, diagnostic tests, lab results, and medicationlists to the physician who is recertifying the patient for hospiceeligibility Makes appropriate recommendations to the IDG based on findingsof the F2F assessment Notifies hospice agency when unable to complete a F2F visitaccording to schedule

About You:

Master’s or higher degree in Nursing. Completion of a graduate-level nurse practitioner educationprogram accredited by a national accrediting body. Current state license as a registered nurse Current state license or certificate as an advanced practicenurse Current national certification as a Nurse Practitioner. (ANCCor AANP) Must have a collaborative agreement in place.
