Pride Health is hiring a Nurse Practitioner in Osceola County, FL! (Contract) Travel between multiple Long Term Care Facilities in
Osceola County Schedule:
Monday-Friday (8am-5pm)
Contract:
7-8 month contract
Pay Rate:
$66-69/hour
Specific Duties:
Provide specialized care to high-risk health plan membersWork as part of an interdisciplinary teamConduct comprehensive assessmentsManage acute and chronic medical and behavioral conditionsEnsure accurate documentation, including ICD-10 codingCommunicate and collaborate with the care teamFacilitate advanced illness and care planning discussionsOffer counseling and education to patients and caregivers
