You will be responsible for the assessment, diagnosis, and treatment of assigned patients for wound care and dermatology.

Responsibilities:

Implement wound care and dermatology treatment plans based on patient assessment in ALF, rehabs, LTACs, and private residences. Maintain accurate patient medical records with comprehensive documentation. Diagnose & establish patient treatment plans. Communicate with physicians or specialists regarding patient care. Monitor & report all clinical changes in wound/skin conditions Perform multiple procedures including bedside debridement, biopsy, I&D, cryotherapy, cauterization, etc. Facilitate referrals to other home health and hospitals Infection prevention Maintain relationships with facilities, home health, and referral sources

Qualification:

Nurse Practitioner or Physician Assistant 2-year prior history of patient care preferred Ability to communicate with patients and facility staff
