We are looking for a qualified Geriatric APRN to join our team and provide primary care health services to our patients in a Senior Living setting. The ideal candidate will have a passion for working with geriatric patients.
Responsibilities:
Provide comprehensive primary care services to residents in Assisted Living, Independent living, and Skilled Nursing communities. Provide excellent and compassionate primary care, helping to improve quality of life for our patients while reducing hospital admissions when possible. Provide direct care to patients with health issues and chronic disease. Assist in developing individualized treatment plans for patients using the collaborative care model. Monitor medications, Home Health needs, and other specialized care. Monitor patient progress and provide feedback to the on-site community team and POA. Maintain patient records and document patient progress.
