Nursing Full Job Description Position Summary:
Brisk MD is seeking a skilled and compassionate Wound Care Nurse Practitioner (NP) to deliver in-home wound care services across both urban and rural communities. The NP will be responsible for evaluating, managing, and treating a wide range of wounds in patients’ homes, promoting healing and improving outcomes through personalized, evidence-based care.
Key Responsibilities:
Perform comprehensive wound assessments and develop individualized treatment plans. Administer advanced wound care treatments, including debridement, dressing changes, and infection control. Monitor wound healing progress and adjust care plans accordingly. Educate patients and caregivers on wound care, prevention, and overall health management. Coordinate with physicians, home health agencies, and other providers to ensure continuity of care. Accurately document clinical findings, treatment plans, and outcomes using an electronic medical record (EMR) system. Travel to patients’ homes in both urban and rural settings, maintaining a professional and compassionate presence. Adhere to clinical guidelines, regulatory standards, and Brisk MD protocols.
Qualifications:
Active and unrestricted Nurse Practitioner (NP) license in the state of Florida. Certification in Wound Care (e.g., WCC, CWCN, CWS) preferred or willingness to obtain. Minimum 1-2 years of clinical experience in wound care, home health, or related field. Excellent clinical judgment, communication, and interpersonal skills. Proficient with EMR systems and mobile documentation tools. Reliable transportation and valid driver’s license required. Ability to work independently in a variety of home environments.
