Ideal candidates will play a critical role in providing high-quality inpatient care. This position requires a strong foundation in patient assessment, triage, and discharge planning, as well as the ability to perform some procedures. The APP will work collaboratively with physicians and other healthcare professionals to ensure optimal patient outcomes. Duties Conduct comprehensive patient assessments to determine health status and care needs. Develop and implement individualized care plans, including discharge planning for patients. Monitor patient progress and adjust treatment plans accordingly. Provide health coaching to patients and their families to promote wellness and disease prevention. Ensure adherence to infection control protocols in all aspects of patient care. Maintain accurate and up-to-date medical records in compliance with healthcare regulations. Collaborate with interdisciplinary teams to enhance patient care delivery. Qualifications Current Nurse Practitioner or Physician Assistant licensure in the state of Georgia. Experience in Critical Care is preferred but not required. Strong skills in patient assessment, discharge planning, triage, and health coaching. Proficiency in basic life support techniques. Knowledge of infection control practices and procedures. Excellent communication skills, both verbal and written, with the ability to work effectively within a team environment.
