Jackson/Camden/Selver/Dyersburg/NashvilleHickory Plaza/Savannah Wayne Road,TNAppleGate Recovery, a divisionof BayMark Health Services, is a multi-site, multi-serviceorganization providing opioid use disorder treatment andrehabilitation for patients across the country. Providing highquality, patient-centered treatment for addiction to prescriptionmedications and other opioids remains the focus of Applegate’sclinics. Combining evidence-based, medication assisted treatmentwith counseling and behavioral services, AppleGate offers the mostsuccessful option for many patients.

Essential Roles andResponsibilities

• Knowledge of various substances of abuse, the disease ofaddiction and signs and symptoms of detoxification
• Collects and records data such as complete HPI and Assessmentand Plan for each patient in a comprehensive, accurate and systemicmanner. Counsels patients and helps them understand their limitationsand abilities
• Documents patient progress/ regress according to establishedpolicies and procedures during appointments or upon notification ofchange in status from clinical coordinator or treatment centerdirector. Supervising and instructing patients in oral medicationself-administration and health education as needed Knowledge of program policies and procedures

• Certifications:

  Must be licensed in the state of Tennessee Experience in MAT/Addiction/Substance Abuse Treatment ispreferred by not required Satisfactory drug screen and criminal background check Must be able to interact compassionately with a diversepopulation
