Travel Nurse PractitionerMonogram Health is lookingfor a skilled Nurse Practitioner eager for the opportunity to makea difference in patients lives. The Monogram Travel NursePractitioner plays a critical role in providing access tocomprehensive care for underserved patients our Monogram Healthnetwork reach. Also, helping patients improve their quality of lifein the home and slow the progression of kidney disease and multipleother chronic illnesses, enabling positive health outcomes.

As a Travel Nurse Practitioner, you will be responsible forthe delivery of personalized and compassionate care to patientsprimarily with CKD through ESRD. Monograms Travel NPs will becomenationwide float providers to the members in areas where our localclinicians are unable to reach and in areas where we are rapidlygrowing. Primary duties include patient health assessment, creatingstrategies to improve or manage a patient’s health, and introducinghabits for health promotion. In part of completing this care forpatients, your responsibilities will include maintaining accuratepatient records, scheduling, and administering follow-upappointments to patients as required across multiplemarkets.
