The ideal candidate will provide comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of chronic conditions. As an FNP, you will play a crucial role in delivering high-quality patient care while collaborating with other healthcare professionals to ensure optimal outcomes. Duties Conduct thorough patient assessments and diagnostic evaluations to determine health status and needs. Develop and implement individualized care plans based on patient evaluations and evidence-based practices. Provide treatment for acute and chronic illnesses, including but not limited to infection control and behavioral health issues Educate patients and families on health maintenance, disease prevention, and management strategies. Collaborate with multidisciplinary teams to coordinate care for patients, ensuring continuity of services. Document all patient interactions accurately in electronic health records (NexGen) to maintain comprehensive medical histories. Experience A Master’s degree in Nursing from an accredited program is required. Certification as a Family Nurse Practitioner is essential. Experience in toddler care is preferred but not mandatory. Familiarity with infection control protocols and case management practices is highly desirable. Strong communication skills and the ability to work collaboratively within a healthcare team are essential for success in this role.
