Position:

The Nurse Practitioner works in collaboration with a physician in an active practice to deliver end of life care and palliative care. The person in this position also serves in a consultant role, acting as an advocate for area hospice programs and providing education for healthcare professionals. The purpose of this position is to provide a bridge between the hospitals and hospices causing improved care for all patients who are terminally ill and provide better management of hospice patients.

Minimum Qualifications:

Edu cation: Graduate from an accredited school of nursing, and a Master’s degree in Nursing.

Licensure/Certifications:

Must hold a current Georgia license for Advanced Practice Nursing.

Must be certified as either:

Gerontologic Nurse Practitioner by the American Academy of Nurse Practitioners Certification Program, or the American Nurses Credentialing Center; or Family Nurse Practitioner or Adult Nurse Practitioner by the American Academy of Nurse Practitioners Certification Board or the American Nurses Credentialing Center. Preferred – Certificate of Added Qualification in gerontology, elder care or care of the older adult provided by the American Academy of Nurse Practitioners, the American Nurses Credentialing Center or a national nurse practitioner certification board. Must be

CPR Certified Previous Experience:

Preferred – Minimum of two years in the management of Medicare patients. Previous experience with home-based medical care is a plus

Knowledge/Training:

Knowledge of Medicare/Medicaid and other funding source regulations as applied to home health care services. Demonstrates knowledge and competence in advanced practice clinical skills, including the nursing process and utilization management.

Computer Skills:

Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs
