Description Part-Time Nurse Practitioner – Lake Charles &Oakdale, LA AppleGate Recovery, a division of BayMark HealthServices, is a multi-site, multi-service organization providingopioid use disorder treatment and rehabilitation for patientsacross the country. Providing high quality, patient-centeredtreatment for addiction to prescription medications and otheropioids remains the focus of Applegate’s clinics. Combiningevidence-based, medication assisted treatment with counseling andbehavioral services, AppleGate offers the most successful optionfor many patients.

Schedule:

9am-1pm (Tues & every other Friday).May need to travel once a week to Oakdale on Tuesdays. EssentialRoles and Responsibilities Knowledge of various substances ofabuse, the disease of addiction and signs and symptoms ofdetoxification Collects and records data such as complete HPI andAssessment and Plan for each patient in a comprehensive, accurateand systemic manner. Counsels patients and helps them understandtheir limitations and abilities Documents patient progress/ regressaccording to established policies and procedures duringappointments or upon notification of change in status from clinicalcoordinator or treatment center director. Supervising andinstructing patients in oral medication self-administration andhealth education as needed Knowledge of program policies andprocedures Qualifications | Education |

Certifications:

Graduate ofan accredited nurse practitioner program (AANP) Must have at least6 months of Addiction Medicine and Primary Care experience required. Must be currently licensed in the state of Louisiana Drugrehabilitation training/knowledge required Satisfactory drug screenand criminal background check.
