Responsibilities The Nurse Practitioner delivers annual risk assessment in aresidential setting or telehealth, within the scope of practice fora Nurse Practitioner, as delegated by the Collaborative Physician.

Essential Duties and Responsibilities Conduct comprehensive in-home health risk assessments toidentify all active and chronic disease conditions, as well asdetermine all physical, mental, and social needs present at thetime of the visit Takes history, examines, determines diagnoses. Provides written documentation of patient visit, per NCQAstandards Takes patient vital signs, as necessary. Places case management referrals and communicates with PCP asnecessary. Communicates with patients, caregivers, agency nurses, otherproviders and vendors as necessary to assure proper diagnosis. Performs all clinical duties while observing OSHA UniversalPrecautions Maintains patient confidentiality Attends required meetings and in-services and participates incommittees, as requested Participates in professional development activities andmaintains professional licenses and affiliationsIn this role youmay work with .
