Responsibilities Seeking a full-time physician trained in Geriatrics, Internal Medicine, or Family Medicine to serve as the primary care provider for medically complex seniors in the PACE model. This is a collaborative, patient-centered position where you’ll work closely with a multidisciplinary team to coordinate and deliver comprehensive care.

Setting:

Outpatient, nursing home, and home care coordination

Schedule:

Full-time; no call or weekends required

Team Collaboration:

Daily IDT meetings, integrated care planning

Responsibilities Include:

Primary care visits and chronic condition management Acute illness evaluations Nursing home rounding and care coordination Integration of specialty care and hospital oversight Quality improvement through data review and clinical collaboration Qualifications MD/DO with board certification in Geriatrics, Internal Medicine, or Family Medicine Minimum 1 year experience working with frail or elderly populations Must be interested in permanent employment Not accepting visa candidates or locums
