Brief Summary of Purpose:
Serves as a float Nurse Practitioner toeach of the Summit ElderCare sites with assignment based onbusiness need. In this role, provides primary health care to SummitElderCare program participants focusing on disease prevention,wellness and management of chronic and episodic illness incollaboration with the program’s Primary Care Providers. Works inclose collaboration with the program multi-disciplinary team toestablish realistic goals and develop and manage individualizedplans of care for program participants. The role may involveworking with Community based Physicians who are enrollingparticipants in the PACE program through the Community-basedPhysician waiver.
Responsibilities Responsibility Summary:
- Performs initial and periodic assessments of participants andappropriately identifies and documents problems/issues in medicalrecords.
- Evaluates episodic illnesses and issues appropriatediagnostic and therapeutic orders.
- Collaborates with physiciansand other multi-disciplinary team members to manage episodic andchronic medical needs of participants.
- Performs diagnostic,therapeutic and nursing interventions as indicated.
- Performsroutine monitoring of chronic, stable illnesses.
- Implements plansof care in a safe and effective manner.
- Participates inmulti-disciplinary team meetings.
- Evaluates effectiveness ofpreviously implemented treatment.
- Conducts home and nursing homevisits as indicated.
- Facilitates the continuum of care forsub-acute and long-term nursing home residents as indicated
- Maintains precise, accurate documentation of all services providedaccording to professional standards.
- Identifies utilization andservice delivery problems and participates in their resolution.
- Participates in quality assessment activities including but notlimited to record reviews, outcome measurement, and review/revisionof policies and procedures.
- Provides education and training toparticipants, caregivers and staff in relevant topics.
- Acts as aresource to program staff and participates in defining nursingstandards and practices.
- Serves on appropriate committees;participates in staff meetings.
- Enhances professional growth andself-development through participation in research activities,other professional activities, educational programs, currentliterature, in-service programs and workshops.
- Maintains BLScertification and professional certification as appropriate.
- Hasresponsibility as a full member of the Summit ElderCare Departmentof Medicine.
- Provides on call coverage for the Summit ElderCareprogram on an as-needed basis or as assigned by the MedicalDirector.
- Supports the fundamental mission of the PACE program.
Qualifications Education:
- Master’s degree required.
Graduate ofan accredited Nurse Practitioner program
License/Certifications:
- Licensed by the Commonwealth of Massachusetts Board of Registrationin Nursing. Valid driver’s license and clean driving recordrequired.
- National Board Certification as a Geriatric, Adult, orFamily Nurse Practitioner.
Experience:
- One to three years ofNurse Practitioner experience preferred with at least one year ofexperience in geriatric care.
- Demonstrates a broad knowledge ofmedical conditions and appropriate interventions.
- Able to applythe principles, methods and techniques of professional nursingpractice to the care of program participants.
- Able to establishand maintain effective working relationships with programparticipants, family caregivers, medical staff and program staff.
