Job Description

Family Nurse Practitioner New Graduate Bradford Healthcare Livermore, ME 04253 Located in Western Maine; this FQCHC offers a rural lifestyle while being close to urban activities. This Family Nurse Practitioner will be part of a patient centered medical home model that believes in quality of care over #s of patient seen. They provide an integrated behavioral health, care managers, and patient assistance coordinators. The providers treat a rural underserved population treating all ages from Newborn to Geriatrics. The FNP will treat acute and chronic health issues, school and sports physicals, women’s health, new baby physicals and small office procedures.

The FNP responsibilities will include:

The Family Nurse Practitioner will treat acute and complex medical issues, The Family Nurse Practitioner will perform school and sport physicals The Family Nurse Practitioner will prescribe and manage medications as needed. The Family Nurse Practitioner will see on average 18 patients per shift and The Family Nurse Practitioner will manage a patient panel of all age groups.
