Nurse Practitioner:

– We are seeking to contract a Nurse Practitioner in Hazleton, PA tohelp ensure our service members get the best care possible. Youwill play a vital role in the continued care of these men andwomen, all while growing your current practice. Your Role at Jamshid Tamiry, M.D., P.C., dba

QTC Medical Group ofAlaska:

As a contracted Nurse Practitioner with Jamshid Tamiry, M.D., P.C.,dba QTC Medical Group of Alaska a Leidos QTC Health Serviceaffiliated professional corporation, you will conduct a variety ofgeneral medicine, occupational, and disability health examinationsincluding, but not limited to: Reviewing medical history and associated records Interpreting clinical data Completing written reports and generating independent medicalopinions (IMO) The examinations may be completed in-person (in a clinic setting,whether it is your clinic, or our clinic), in the examinee’s home,at large event settings for the Reserve Health ReadinessProgram.

Primary Responsibilities:

Conduct physical exams for a variety of customers to includefederal, defense and others. Occupational health exams to include pre- and post-employmentand annual physicals. Written medical opinions based on provided medicalrecords/information Unique exam settings – homebound, event sites, etc. Completion of Veterans Benefits Administration DisabilityBenefit Questionnaires (DBQs) Provide primary medical evaluations to include, but not limitedto review of systems, pap smear, and range of motion Complete initial review and interpretation of diagnosticstudies to include but not limited to laboratory, pulmonaryfunction and ECG studies Documentation of examinee records in appropriatesystems. Collaboration with other professional and clinical staff asneeded and communicate with internal and client team members indetermining the most effective and evidence-based way forward onbehalf of the examinee population.

Required Qualifications:

Education:

Master’s Degree in Nurse Practitioner Studies from anaccredited university or college Completed Education and training in an US accredited,post-baccalaureate (Masters) –

Experience:

Minimum 2 years of post-graduate experience in General Medicineor Internal Medicine. Minimum 3 years of post-graduate primary care experience

Certification:

Valid

BLS OR ACLS

certification Unrestricted State medical licensure without limitations toperform full scope of authorized practice National Licensing with ANCC or AANP Valid DEA license Valid CPR or BLS certification

Clearance:

Ability to pass a medical clearance Must be able to obtain and maintain government clearances(LARS, Public Trust, NACI, BGI, etc.) as required.

Preferred Qualifications:

Experience conducting occupational/disability medicalexaminations

Work Schedule and Compensation:

Location:

Hazleton, PA.

Flexible Schedule:

Appointments are typically scheduledapproximately 5 to 7 days of notification. You set the pace andfrequency!

Compensation:

Compensation will be provided for eachexamination, with payment determined based on the specificrequirements and type of examination performed.

Additional Information:

Provider must complete required online training courses, up to13 courses at approx. 17.5 hours total. Leidos QTC Health Services will run a 7-year criminal historyand sex offender background check. Once the BGI and credentialing have been completed, theProvider will be required to obtain CPEP Certification (approx.17hours/depending on specialty) You will be given access to the Provider Liaison (PL)department; this department assists with any issues, questions, orneeds you may have while contracted with Leidos QTC HealthServices. –
