Optum is seeking a HouseCalls Part Time 20-hour, StatewideTraveler, Missouri and Iowa. - Missouri and Iowa
- ,approximately 85% of the time.
All travel expenses are covered, andthere is likewise a base salary plus multiple incentives.
PrimaryResponsibilities:
- Conduct in-home assessments including anevaluation of past medical history, review of systems, medicationreconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screenings including point of caretesting (as appropriate)
- Identify diagnoses to be used in caremanagement and active medical management in the furtherance oftreatment
- Formulate a list of current and past medical conditionsusing clinical knowledge and judgment and the findings of yourassessment
- Communicate findings in your assessment that will beused to inform member’s PCP of potential gaps in care
- Identifyurgent and emergent situations and intervene appropriately
- Educate members on topics such as disease process, medication, andcompliance You’ll be rewarded and recognized for your performancein an environment that will challenge you and give you cleardirection on what it takes to succeed in your role as well asprovide development for other roles you may be interested in.
Required Qualifications:
- Active NP license or ability to obtainby start date. Licensure must be unencumbered, free of anyopen/unresolved disciplinary actions including probation orrestrictions against privilege to practice

NPs:
APCs working injurisdictions that authorize APCs to practice autonomously orwithout formal supervision must have obtained approval to practiceautonomously or without formal supervision from their licensingboard, if applicable. New hires who are eligible and have notapplied prior to hire date, must apply to practice autonomously orwithout supervision within 1 month of hire. If not eligible topractice autonomously or without formal supervision at hire, theAPC must begin working towards meeting the requirement within 1month of hire, if applicable, and apply for approval to practiceautonomously or without formal supervision within 3 months ofbecoming eligible; or
PAs:
Ability to work under a CollaborativeAgreement in applicable states
- Active ANCC or AANP nationalcertification in Family, Geriatrics, Adult, Adult-GerontologyPrimary Care, or Adult-Gerontology Acute Care certification, or theability to obtain national certification and/or NP license in stateof assignment by start date
- Ability to complete physicalrequirements of the job including lifting a 30-pound bag in and outof the car, navigating stairs and a variety of dwelling conditions,and sit, stand and kneel to perform physical assessment
- Access toreliable transportation to travel up to 85% within licensed states(with notice, based on business needs) to complete home visitassessments
Preferred Qualifications:
- 1 years of clinicalexperience in their highest level of education, clinical setting
- Experience in Gerontology, Cardiology, Internal medicine, orEndocrinology
- Home Health care or home visit experience
- May berequested to obtain additional licensure in other geographic areasThe salary range for this role is $89,800 to $176,700 annuallybased on full-time employment.

PLEASE NOTE
Employees must be in anactive regular status. Employees must remain in role for a minimumof 12 months from the date of hire /rehire/transfer. If an employeeleaves Home and Community, the student loan repayments will cease.
The employee must remain in an Advanced Practice Clinician orPhysician role within Home and Community for 36 months to receivethe full benefit of the student loan repayments.
