Optum is seeking a Midwest Regional Traveler Nurse Practitioner tojoin our HouseCalls team in the Midwest to cover KS, MO, IA, MN,NE, ND, SD and WE. No on-call, no weekends and noholidays required. This role requires multi-state travel across theMid-West of the United States, approximately 85% of the time. Alltravel expenses are covered, and there is likewise a competitivebase salary plus multiple incentives.
Primary Responsibilities:
- Conduct in-home assessments including an evaluation of past medicalhistory, review of systems, medication reconciliation, vital signs,and comprehensive physical exam
- Perform evidence-based practicescreenings including point of care testing (as appropriate)
- Identify diagnoses to be used in care management and active medicalmanagement in the furtherance of treatment
- Formulate a list ofcurrent and past medical conditions using clinical knowledge andjudgment and the findings of your assessment
- Communicate findingsin your assessment that will be used to inform member’s PCP ofpotential gaps in care
- Identify urgent and emergent situationsand intervene appropriately
- Educate members on topics such asdisease process, medication, and compliance You’ll be rewarded andrecognized for your performance in an environment that willchallenge you and give you clear direction on what it takes tosucceed in your role as well as provide development for other rolesyou may be interested in.
Required Qualifications:
- Active NPlicense or ability to obtain by start date. Licensure must beunencumbered, free of any open/unresolved disciplinary actionsincluding probation or restrictions against privilege to practice

NPs:
APCs working in jurisdictions that authorize APCs to practiceautonomously or without formal supervision must have obtainedapproval to practice autonomously or without formal supervisionfrom their licensing board, if applicable. New hires who areeligible and have not applied prior to hire date, must apply topractice autonomously or without supervision within 1 month ofhire. If not eligible to practice autonomously or without formalsupervision at hire, the APC must begin working towards meeting therequirement within 1 month of hire, if applicable, and apply forapproval to practice autonomously or without formal supervisionwithin 3 months of becoming eligible; or
PAs:
Ability to work undera Collaborative Agreement in applicable states
- Active ANCC orAANP national certification in Family, Geriatrics, Adult,Adult-Gerontology Primary Care, or Adult-Gerontology Acute Carecertification, or the ability to obtain national certificationand/or NP license in state of assignment by start date
- Ability tocomplete physical requirements of the job including lifting a30-pound bag in and out of the car, navigating stairs and a varietyof dwelling conditions, and sit, stand and kneel to performphysical assessment
- Access to reliable transportation to travelup to 85% within licensed states (with notice, based on businessneeds) to complete home visit assessments Preferred Qualifications
- May be requested to obtain additional licensure in othergeographic areas
- 1 years of clinical experience in their highestlevel of education, clinical setting
- Experience in Gerontology,Cardiology, Internal medicine, or Endocrinology
- Home Health careor home visit experience
PLEASE NOTE
Employees must be in an activeregular status. Employees must remain in role for a minimum of 12months from the date of hire /rehire/transfer. If an employeeleaves Home and Community, the student loan repayments will cease.
The employee must remain in an Advanced Practice Clinician orPhysician role within Home and Community for 36 months to receivethe full benefit of the student loan repayments. The salary rangefor this role is $89,800 to $176,700 annually based on full-timeemployment. Pay is based on several factors including but notlimited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
