SUMMARY We are seeking a compassionate and skilled Nurse Practitioner orPhysician Assistant to join our healthcare team, deliveringhigh-quality primary care in senior living facilities. In thisrole, you will provide direct patient care, focusing on promotingwellness, managing chronic conditions, and coordinating care withother healthcare providers. ESSENTIAL DUTIES AND RESPONSIBILITIES
Patient Care and Assessment:
Perform comprehensive assessments,develop treatment plans, and provide ongoing care management forresidents, focusing on preventive care and chronic diseasemanagement.
Routine Rounds and Monitoring:
Conduct regular rounds to assessresidents’ conditions, adjust treatment plans, and address anyemergent health concerns.
Care Coordination:
Collaborate with facility staff, family members,and other healthcare providers to ensure comprehensive, coordinatedcare tailored to the residents’ needs.
Patient Education:
Educate residents, families, and facility staffon disease prevention, wellness, and managing chronic illnesses tosupport better health outcomes.
Documentation and Compliance:
Maintain accurate, timely, andthorough documentation within the electronic medical record (EMR)system, ensuring compliance with regulatory standards andorganizational protocols.
Quality Assurance:
Participate in quality improvement initiativesto enhance care delivery, reduce hospital readmissions, andoptimize patient satisfaction within the facility.
QUALIFICATIONS
Valid nurse practitioner or physician assistant license in state ofoperations & current board certification in your field Masters Degree in Nursing or Physician Assistant Studies Strong leadership, interpersonal, and problem-solving skillspreferred. Must possess excellent written and oral communication skills Must possess traits of punctuality, organizational skills, andability to function within a team.
