- This role requires travel across the states of Idaho and Montana,approximately 85% of the time. All travel expenses are covered, andthere is likewise a base salary plus multiple incentives.

Primary Responsibilities:
Conduct in-home assessments including an evaluation of pastmedical history, review of systems, medication reconciliation,vital signs, and comprehensive physical exam Perform evidence-based practice screenings including point ofcare testing (as appropriate) Identify diagnoses to be used in care management and activemedical management in the furtherance of treatment Formulate a list of current and past medical conditions usingclinical knowledge and judgment and the findings of yourassessment Communicate findings in your assessment that will be used toinform member’s PCP of potential gaps in care Identify urgent and emergent situations and interveneappropriately Educate members on topics such as disease process, medication,and compliance

RequiredQualifications:
Active NP license or ability to obtain by start date. Licensuremust be unencumbered, free of any open/unresolved disciplinaryactions including probation or restrictions against privilege topractice Active ANCC or AANP national certification in Family, Geriatrics,Adult, Adult-Gerontology Primary Care, or Adult-Gerontology AcuteCare certification, or the ability to obtain national certificationand/or NP license in state of assignment by start date Ability to complete physical requirements of the job includingmoving a 30-pound bag in and out of the car, navigating stairs anda variety of dwelling conditions, remain in a stationary positionand position self to perform physical assessment Driver’s license and access to reliable transportation to travel upto 85% within licensed states (with notice, based on businessneeds) to complete home visit assessments

Preferred Qualifications:
1+ years
- of clinical experience in their highest level ofeducation, clinical setting
- Experience in Gerontology, Cardiology, Internal medicine, orEndocrinology
- Home Health care or home visit experience
- May be requested to obtain additional licensure in othergeographic areas

PLEASE NOTE
- Employees must be in an active regular status.
Employees must remain in role for a minimum of 12 months from thedate of hire /rehire/transfer.
- If an employee leaves Home andCommunity, the student loan repayments will cease.
- The employeemust remain in an Advanced Practice Clinician or Physician rolewithin Home and Community for 36 months to receive the full benefitof the student loan repayments.
- The salary range for this role is $89,800 to $176,700 annuallybased on full-time employment.
