About the Role Provide comprehensive primary care services 4 days per week Collaborative with interdisciplinary teams to optimize patient outcomes Ensure smooth transitions of care with hospice, palliative care, and home care agencies Maintain accurate and timely documentation in our EMR system (Gehrimed) Qualifications Graduate degree as a nurse practitioner or physician assistant Active and unrestricted NP or PA license in North Carolina Strong clinical assessment and diagnostic skills Excellent communication and interpersonal abilities Experience in post-acute care preferred, but not required (training available) Clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family, primary, internal, adult, post-acute, home health, hospice, and hospital medicine. New grads are welcome! Compensation is fee for service (FFS). Expected compensation is estimated salary range of $82,000 to $98,400 with no cap on productivity income potential.
