NURSE PRACTITIONER/PHYSICIANASSISTANT PART TIME
(24 hours/week)
Job Description:
Providesclient services to local Skilled Nursing & Rehabilitationfacilities. Assures quality nursing care for patients by directingand coordinating nursing care. Conducts physicals and annualassessments for assigned patients. Completes acute visits to assessand diagnose patient conditions. Focuses on early identification ofchanges in condition as well as strong management of chronicconditions. Completes history and physical on new admissions within72 hours of admission or readmission. Completes Medicarecertifications according to required time frame. Prepares dischargeorders, instructions and prescriptions for patients beingdischarged. Encourages advanced care planning and end of lifedecisions for patients requiring these services. Provides pharmacywith needed narcotic prescription refills as patient need dictates.
Documents patient assessment and plan of care in the electronicmedical record. Reviews labs and other diagnostic tests foradditional interventions. Collaborates with physicians and staff.
Notifies family of changes in condition. Communicates daily withnursing staff, assigns duties, supervises, and evaluates workperformance. Reviews nursing policies and procedures forcompliance, and to make recommendations for changes/additions asnecessary. Recommends or conducts specific areas of in-service orcontinuing education. Assesses patients for palliative care needsand creates/oversees the palliative plan of care In facilities andin patient’s homes.
Job Requirements:
Current, unrestricted validNurse Practitioner license from the Board of Nursing/PA Licensefrom the Medical Board in the state of assignment or the ability toobtain. DEA certificate current or the ability to obtain. Clinicalbackground in adult, family or geriatric specialties preferred.
Past experience working in long-term care, a nursing home, or withseniors in other settings preferred. Strong communication andrelationship building skills preferred. Intermediate level ofproficiency in PC based word processing, including Microsoft Wordand Outlook. Willing to be ‘on call’ for Facilities as needed.
Dependable, flexible, and able to work and cooperate well with allnursing personnel and other departments. Ability to make decisionsregarding nursing problems, work well under pressure, and problemsolve. Background checks/drug-free workplace. EOE.PI8f427d12885e-37456-36591051
