Eventus WholeHealth delivers an integrated model of care toadults who reside in skilled nursing and assisted living facilitiesthrough a network of healthcare providers including Physicians,Nurse Practitioners, Physician Assistants, Medical Assistants andin-house Support Staff.
Essential Duties And ResponsibilitiesProvideprimary-care services in accordance with Eventus WholeHealthsite-specific protocols and consistent with the standard of carefor the specialty of family medicine, geriatric medicine, and/orinternal medicine as it pertains to long term care facilitywork.
Quality/Compliance Review and sign MAR’s as requested by facilities, sign allorders specific for Primary Care while at the facility and/orwithin 24 hours if faxed or emailed to include phone orders, DMEorders, Death Certificates, Plans of Care, PT/OT orders, hospiceorders as appropriate, and other orders as is relevant andappropriate to the facility patient care needs and EventusWholeHealth. Review and address labs within 24 hours of them beingsent to you. Manage all narcotic prescriptions specific to yourfacility’s patients and as per Eventus WholeHealth Narcotic RxPolicy. Provide facility with adequate time each visit to allow forpaperwork administrative duties. Complete documentation and submit billing records to EventusWholeHealth according to the published policies on charting andbill sheet submission and provide the facility with a hard copy ofthe electronic documentation within 48 hours of service forinclusion in the patient’s facility medical record. Establish and maintain open and positive communications withfacility staff and administration. Provide verbal and/or writteninstruction or feedback regarding medications and other pertinentcaregiver information. Meet with Administrator or designatedfacility care coordinator equivalent prior to the start of yourpatient care visits each day and prior to leaving facility eachday. Be knowledgeable of and adhere to Eventus WholeHealthstandards, policies and procedures. Be aware of and adhere to alllegal and regulatory agencies’ rules and guidelines andprofessional ethical standards. Comply with all regulatory agenciesgoverning health care delivery. Maintain patient confidentiality at all times includingappropriate use of Cell phone, emails text messaging, patientcharts and EMR. Agrees to abide by and be knowledgeable of HIPAArules and regulations Consult with the clinical supervisor before refusing aresident, family, or facility request or recommendation regardingthe care of a resident.
Qualifications/Education Masters degree in Nursing from an accredited educationalinstitution and a current and valid license to practice medicineissued by the state in which you reside Medical Board. All othercertification necessary to perform one’s duties must be current andvalid, required.
OR Have graduated from a physician assistant educational programthat is accredited by a National Commission on Accreditation ofAllied Education Programs. Have passed the national certification examination of theNational Commission on Accreditation of Certification of PhysicianAssistants. Licensed by the state in which you reside as a PhysicianAssistant. Specialization in geriatric, family, or psychiatry desired.
