Job Location Duke University Hospital Job SummaryThe Duke Hospital Pediatric Critical Care Medicine Cardiac StepdownAdvanced Practice Provider (APP), including both Nurse Practitioner(NP) and/or Physician Assistant (PA), provides direct patient careservices for critically ill pediatric patients aged 0-21 withacute, complex, and chronic illness across a variety of caresettings. It is appropriate for the NP to provide care for an agingpediatric patient beyond the age of 21 with a pediatric conditionfor a limited period of time. Pediatric Cardiac Stepdown APPs willbe a part of the Pediatric Critical Care Medicine team and willcare for patients on the stepdown unit. Utilizing acquiredknowledge of physiology, pathophysiology and pharmacology, the PCCMAPP exercises independent decision making in the assessment,diagnosis and development of the plan of care for patients on thePCCM service. In collaboration and consultation with thesupervising physician, the
PCCM APP
contributes to excellence inpatient care. As an advanced practice provider, the
PCCM APP
isadditionally involved in education, consultation, research andleadership throughout the Duke Health System, organization andprofession. Job Duties and Responsibilities Clinical Expert
- Incollaboration with attending(s) and/or fellow/residents, rounddaily on patients to assess, diagnose, plan treatment, setpriorities and realistic outcomes, and evaluate the effectivenessand cost efficiency of patient care.
- Provide and coordinateclinical care for service specific patients through establishedprotocols and under the supervising physician or back up physicianwho is either on site or available by pager.
- Collaborate with theresidents, fellows, attending, other APP colleagues, nurses, andother staff to formulate treatment plans and monitor patientprogress.
- Perform procedures specific to the work environmentafter demonstrated competency and within the guidelines of scope ofpractice and credentialing protocols.
- Assess patients for changein status and institute appropriate interventions. Initiateemergency interventions in an effort to stabilize the patient
- Postoperative stabilization and management.
- Develop patient carepathways, protocols and guidelines for the management of specificproblems in conjunction with physicians and other members of thehealth care team.
- Analyze clinical practice patterns to providethe best medical care and to increase effectiveness and efficiency
- Communicate verbally and in writing all pertinent patientinformation to supervising MD, nursing staff, patient resourcemanager, home health agency, and other relevant healthcareproviders.
- Document in the patients’ medical record all relevantdata including but not limited to results of diagnostic tests;laboratory results; patients’ condition and response totherapies/interventions; communications with supervising MD.
- Utilize prescriptive privileges only for identified patientpopulation.
- Prescriptive authority for prescribing, ordering, andor administering drugs; including (but not limited to) schedule II,III, IV, and V drugs and necessary medical devices.
- Role modelcompetence in all nurse practitioner / physician assistant advancedskills.
- Give daily handoff to appropriate attending, APP, fellow,resident and/or intern at the end of the shift.
- Provide evidencebase clinical care to pediatric patients cared for by the divisionof PCCM
- Gather pertinent information systematically andskillfully from all sources: History, diagnostic tests,comprehensive physical exams, behavioral and developmentalassessments.
- Bill appropriately for all APP services.
Educator:
- Assess educational needs of the patient-care staff and provideone-on-one as well as formal education as needed.
- Assesseducational needs of the patients / families and provideculturally-sensitive patient/family education interventions asindicated. Use non-technical language when educating patients andfamilies. Document all education in patients’ medical record.
- Implement and evaluate appropriate educational programs for DUHSpatient care staff and other healthcare providers as appropriate.
- Role model and precept healthcare learners through contractualagreements.
Research Utilization and Investigation:
- Utilizecurrent research and evidence-based decision-making in all clinicalpractice.
- Incorporate clinical research findings in thedevelopment and implementation of standards of care.
- Support IRBapproved clinical research/trials though a variety of activitiesincluding but not limited to: obtaining consent; orderingdiagnostic tests; recording laboratory & diagnostic results;administering therapeutic interventions; and reporting patientoutcomes including toxicities or adverse events.
- Participate ininterdisciplinary quality improvement and/or research activities.
- Participate in measuring and documenting outcomes.
- Identifypotential areas for research
Consultant:
- Provide expertise andresource information to other healthcare providers, patients andfamilies, and the community at large related to pediatric diseases,procedures, interventions, and conditions.
- Implement and evaluateappropriate educational programs for referring MDs
- Develop,implement and evaluate standards of care/practiceguidelines/policies and procedures/care maps/protocols within areaof specialization.
- Develop programs and projects in areas ofexpertise to facilitate the attainment of the goals and objectivesof the Duke University Hospital and Duke University Health System

Professional Leadership and Development:
- Role model advancedpractice professionalism through conduct, communication, dress,leadership, ethical decision-making, critical thinking and problemsolving skills.
- Participate in at least one professionalorganization.
- Demonstrate responsibility for professionalpractice through active participation in the CAP, and professionalorganizations. Meet Continuing Education requirements as designatedby the NC Medical Board and/or NC Nursing Board. Maintainprofessional certification.
- Enhance the body of knowledge in areaof specialization through written publications, oral presentations,posters, continuing education, etc.
- Maintain all professionalrequirements for licensure and certification
- Attend educationalconferences such as Heart Center Surgical conference, cardiologylectures, and neurodevelopmental rounds
- When caring for adultswith congenital heart disease, plan for situations beyondexpertise, consult with and refer to other health care providers asappropriate.
Job Eligibility Requirements Education:
NP:
Graduateof MSN or DNP Pediatric Acute Care Nurse Practitioner Program.
PA:
Graduate of Master’s or Doctoral Degree ARC accredited PhysicianAssistant program.
Licensure:
NP:
Current unrestricted RN licenseissued by or recognized by the NCBON with approval to practice asan NP.
PA:
Current unrestricted medical license issued by NCMB withapproval to practice as a PA.
Certification:
NP:
PNP-AC is required

PA-C Experience:
Prior experience as a NursePractitioner/Physician Assistant in a pediatric acute care clinicalsetting strongly preferred with preference for a cardiac-specificbackground. Registered Nurses with at least 2 years of ICU nursingexperience preferred. Physical and mental requirements
- Able toclearly articulate scope of practice, and practices within thoseguidelines as a Nurse Practitioner / Physician Assistant in thestate of North Carolina and within the Duke University HealthSystem.
