Primary Responsibilities:
- Conduct in-homeassessments including an evaluation of past medical history, reviewof systems, medication reconciliation, vital signs, andcomprehensive physical exam
- Perform evidence-based practicescreenings including point of care testing (as appropriate)
- Identify diagnoses to be used in care management and active medicalmanagement in the furtherance of treatment
- Formulate a list ofcurrent and past medical conditions using clinical knowledge andjudgment and the findings of your assessment
- Communicate findingsin your assessment that will be used to inform member’s PCP ofpotential gaps in care
- Identify urgent and emergent situationsand intervene appropriately
- Educate members on topics such asdisease process, medication, and compliance You’ll be rewarded andrecognized for your performance in an environment that willchallenge you and give you clear direction on what it takes tosucceed in your role as well as provide development for other rolesyou may be interested in.
Required Qualifications:
- Active NP orPA license or ability to obtain by start date. Licensure must beunencumbered, free of any open/unresolved disciplinary actionsincluding probation or restrictions against privilege to practice
- For NPs
- Active ANCC or AANP national certification in Family,Geriatrics, Adult, Adult-Gerontology Primary Care, orAdult-Gerontology Acute Care certification OR for PAs
- PhysicianAssistant national certification through NCCPA
- Ability tocomplete physical requirements of the job including lifting a30-pound bag in and out of the car, navigating stairs and a varietyof dwelling conditions, and sit, stand and kneel to performphysical assessment
- Reliable transportation to complete homevisit assessments
Preferred Qualifications:
- 1 years of clinicalexperience in their highest level of education, clinical setting
- May be requested to obtain additional licensure in other geographicareas
- Experience in gerontology, cardiology, internal medicine,or endocrinology
- Home Health care or home visit experience
- Ability to travel throughout the state, beyond your assignedregion, up to 5% of the time.
Based on business needs with advancednotice, eligible for additional compensation incentives PLEASE NOTEEmployees must be in an active regular status. Employees mustremain in role for a minimum of 12 months from the date of hire/rehire/transfer. If an employee leaves Home and Community, thestudent loan repayments will cease. The employee must remain in anAdvanced Practice Clinician or Physician role within Home andCommunity for 36 months to receive the full benefit of the studentloan repayments. The salary range for this role is $89,800 to$176,700 annually based on full-time employment. Pay is based onseveral factors including but not limited to local labor markets,education, work experience, certifications, etc. UnitedHealth Groupcomplies with all minimum wage laws as applicable.
