Job Summary This is a full-time professional position. This position will assist in providing Palliative Care Consultation and Hospice Care to patients in hospitals, residential facilities, Hospice Inpatient Facility and home settings. This position will provide direct patient care on an ongoing basis as directed by the APP Clinical Manager, Chief Medical Officer, the associated Medical Director, the referring physician, the patient and family, and the care plan. This position will work with the APP Clinical Manager and Chief Medical Officer to educate hospital staff, residential facility staff, and community physicians about palliative care, pain and symptom management, communication around end-of-life decision making, and other programs available within the organizations continuum of care (Hospice, Palliative Care, Serious Illness Programs). The schedule for this position is Monday-Friday 8:30am-5:00pm; on-call rotation; holiday rotation. Responsibilities what you do Provide Palliative Care Consultation and Hospice visits to patients in residential facilities, home settings, Hospice Inpatient Facility and hospitals. Provide direct patient care on an ongoing basis as directed by and under the clinical supervision of the APP Clinical Manager, Chief Medical Officer, the associated Medical Director and the referring physician, patient, family, and plan of care; utilizing pain and symptom management interventions that are based on established hospice and/or palliative care guidelines and standards. Provide attending MD status for Hospice patients when assigned. Provide Face to Face visits to ensure compliance with Hospice Medicare/Medicaid regulations. Attend IDG meetings as appropriate Document in patient medical record to meet federal, state, regulatory, and agency guidelines. Work with the APP Clinical Manager, Chief Medical Officer, and associated Medical Director to educate hospital staff, residential facility staff, and the community physicians about palliative care pain, and symptom management, communication around end-of-life decision making and other programs available, i.e. .HospiceCare, PalliativeCare, Serious Illness Programs. Assist in data collection, forms, and outcome evaluation tools for the PalliativeCare Program or HospiceCare Program as needed. Participate in on-call rotation. Adheres to NHPCO Ethical Practices and the American Nurses Association’s Code of Ethics. Requirements what you need Nurse Practitioner with current N.C. license Preferred 3 years clinical experience. Preferred 2 years experience in hospice, palliative care, and/or primary care. Preferred ACHPN certification. Maintain educational requirements to meet Nurse Practitioner standards. Knowledgeable about reimbursements sources, Medicare regulations, licensing laws, and accreditation standards. Computer skills preferred. Current CPR certification. Must have and maintain a valid North Carolina Driver’s License, maintain automobile insurance coverage and have access to reliable transportation.
