Salary:
$110,000 – $140,000 per year A bitabout us: We are a local PACE program (Providing All Inclusive Careto the Elderly) right here in Greensboro, NC. In this role, youwill work as part of our care team, which includes other providers,RNs, CNAs, and Physical Therapists. This position requires anactive and unencumbered NP license. Are you a compassionate,dedicated Nurse Practitioner with a passion for providingexceptional care to the geriatric population? We are seeking anexperienced Nurse Practitioner to join our team in the PACE(Program of All-Inclusive Care for the Elderly) Program. This is apermanent, full-time position that offers the opportunity to workwith a multidisciplinary team focused on providing comprehensivehealthcare services to elderly individuals in our community. Thisis a full time role, 40 hours a week working a Monday throughFriday daylight schedule.
Responsibilities:
As a Nurse Practitionerin our PACE Program, your responsibilities will include: 1.Providing comprehensive healthcare services to PACE participants,including assessment, diagnosis, treatment, and follow-up care. 2.Collaborating with a multidisciplinary team of healthcareprofessionals to develop and implement individualized care plansfor each participant. 3. Educating participants and their familieson disease prevention and health maintenance. 4. Conducting regularhealth assessments to monitor participants’ health status andprogress. 5. Coordinating with other healthcare providers to ensureseamless, integrated care for participants. 6. Participating inPACE team meetings to discuss participants’ care plans andprogress. 7. Adhering to all regulatory standards and bestpractices in geriatric care. 8. Documenting all patient care inaccordance with established standards.
Qualifications:
To qualifyfor this exciting opportunity, you will need: 1. A Master’s degreein Nursing from an accredited institution. 2. Current licensure asa Nurse Practitioner in the state of practice. 3. Certificationfrom the American Nurses Credentialing Center (ANCC). 4. A minimumof 2 years of experience as a Nurse Practitioner, with a focus ongeriatric care proffered. 5. Demonstrated experience with the PACEprogram or equivalent. 6. Exceptional clinical skills with theability to assess, diagnose, and treat a wide range of healthconditions in the geriatric population. 7. Excellent communicationand interpersonal skills, with the ability to work effectively in amultidisciplinary team. 8. Strong organizational and timemanagement skills, with the ability to manage a high caseloadeffectively. 9. A commitment to providing compassionate,patient-centered care to the elderly.
