Overview:
We are looking for a AIMStatewide Travel Nurse Practitioner -to join our team. Thisposition will directly report to the AVP of Advanced IllnessManagement (AIM) and is responsible for facilitating and providingadvanced illness management and palliative care services topalliative care patients. Provide individualized treatmentplans and comprehensive care from the comfort of patients’ home:including providing diagnosis, treatment, consultation, andfollow-up under the direction of the Palliative Medical Director orPrimary Care physician.
Role requires up to 80-90% statewide travel. Requires regular travel betweenpalliative branches throughout the state to provide patient careand other essential responsibilities.
May potentially be someoccasional regional travel if needed (less than 10%).Flex oralternative schedules available.
About You:
Minimum of Master of Science in Nursing from an accreditedschool of nursing, or an equivalent as allowed under state law,i.e. DNP (Doctorate of Nursing) 2+ years Nurse Practitioner experience Current license as a registered nurse and equivalent asrequired by state laws for Nurse Practitioner licensure withoutboard actions. Active and unrestricted Nurse Practitionercertification/license without board actions. – Multi state licensure is desirable for Travel NursePractitioner role or willing to obtain additional state licensure(company paid) for those interested in regional travelopportunities (e.g. assist with new site launches) Practice experience with Part B documentation and coding Must have a collaborative agreement in place with a supervisingphysician agreeable to supervise work in palliative care program ormust agree to enter a collaborative agreement with PalliativeMedical Director, as required by state law. Part B Provider certification Federal DEA license – State DEA license or state specific prescribing authority (ifrequired) ACHPN – Certification in Hospice and Palliative CarePreferred Nurse Practitioner specialty in Family or Adult Medicine, AcuteCare preferred
