$100ResponsibilitiesKnowledge of CMS Regulations and NCQA HEDISGuidelines;
Knowledge of Evidence-based Clinical Practice Standards:
American Diabetes Association (ADA), American College of Cardiology(ACC), American Heart Association (AHA)Familiarity with ICD-9,ICD-10 and CPT-4 coding practices;Complete comprehensive, accurateand thorough review of the assigned member population, includingtimely completion and submission of all required encounterdocumentation (paper or electronic);Ensure that all pertinent andactive medical conditions are documented in the medical record in amanner compliant with CMS/DHHS, Inovalon policy, and clientrequirements;Support the physician/patient relationship and ensuretimely and adequate communication, documentation of assessmentfindings, recommendations, need for additional services, emergencyservices required if necessary and need for follow up and timelinesfor follow up to primary care provider (PCP) and health plan asrequired;Make general recommendations to members intended toimprove members’ knowledge of their chronic condition(s), such asinformation concerning recommended testing;Address and closeidentified gaps in care (disease-specific or preventive); Recognizeemergent or urgent situations requiring escalation and takeappropriate action as specified in Inovalon policies, and asdetermined by reasonable professional judgment and ethicalprofessional practice standards; andPerform, document andcommunicate results of Point of Care (POC) Testing.
